Our office will make every effort to schedule your patients to be seen by one of our providers as soon as possible.
The New Patient Evaluation Form provides an opportunity for patients to choose a specific provider from our practice, if they wish.
We accept new patients by physician referral only. It can be faxed to our office at 330-208-2721. After we receive a written referral from your office and your patient has been scheduled for their first visit, they must arrive to their first appointment with the following:
Current valid photo ID (driver's license, state ID, passport, etc.)
Insurance card(s)
Co-pay if applicable
List of all current medications including herbs and vitamins
Mail or fax medical records pertinent to our services directly to the main office (listed below) regardless of which office your patient may be choosing to utilize. The patient will not be scheduled for his or her appointment until complete medical records are received. Consultation reports are done for all our patients. Our staff will fax reports directly to the fax number specified; we normally respond within one business day.
